A case of paraplegia in a patient submitted to aorto bisiliac thrombo endarterectomy is reported. The rarity of bone marrow lesions during abdominal aorta surgery (18 cases in the literature) leads to a discussion of the pathogenetic details of the disease (anatomical variations in spinal vascularization arteriosclerosis of the bone marrow vessels; operative shock, etc.). The present case, which had a good outcome, was even more significant as the aortic lesion was not an aneurysm, as is generally the case in the literature. Rehabilitation methods are also discussed and possibilities of preventing the complication reviewed.
