Abstract
A case of a 14-year-old nulligravida girl affected with paroxysmal nocturnal hemoglobinuria (PNH) is reported. The patient's phenotype was A1, rr, Le (a-b-), Kk. SHe was repeatedly transfused with only rr red blood cells, but she developed a strong and rapidly increasing anti-D (Rh(o)) antibody. It is suggested that the immunogenic stimulus could have been given by the D (Rh(o)) antigen present on the inner membrane surface of her rr red blood cells, exposed during severe and recurrent hemolytic crises. Laboratory evidence of this suggestion is presented.
