We describe a case of pleuropneumonia which evolved in pleural empyema and required both surgical and medical therapy in a 10 years old child. Because of the great amount of pediatric respiratory system infections, we intended to set out a new diagnostic approach to a early identification of the surgical cases in order to adopt less invasive techniques for the patient and more economic for the community.
