We describe a case of a rare form of hypertrophic cardiomyopathy in a 53-year old man. Peculiar anatomic features of a massive hypertrophy of the anterior and lateral free wall of the left ventricle were clearly suspected on transthoracic echocardiography and precisely recognized through cardiac multidetector computed tomography.
