Adjuvant vinblastine plus bleomycin continuous infusion chemotherapy of testicular carcinomas following radical retroperitoneal dissection

Abstract

Fourteen previously untreated patients with testicular carcinoma metastatic to the retroperitoneal lymph nodes received adjuvant chemotherapy following orchiectomy and radical retroperitoneal dissection (RPLND). Two to three weeks from surgery, patients were administered vinblastine, 6 mg/m 2 on days 1 and 2, followed by continuous intravenous administration of bleomycin, 15 mg/m 2/day, for 5 successive days. This combination was repeated at 4-5 week intervals for 3 cycles. Median follow-up period after RPLND was 17 months (range 8-34). Three of 14 patients relapsed after 2, 6, and 10 months. In all these patients, extracapsular spread was present, while in 2 of them, more than 5 nodes of involvement had been demonstrated at RPLND. To critically evaluate the efficacy of adjuvant chemotherapy in testicular carcinomas, a detailed evaluation of prognostic factors in those series treated with RPLND alone should be performed.
