Adrenergic mechanisms do not contribute to glucose counterregulation during acute insulin-induced hypoglycaemia in normal man

Abstract

Adrenergic mechanisms are usually not critical for glucose counterregulation during acute and moderate, insulin-induced hypoglycaemia in man, as long as the responses of plasma glucagon are normal. To test the hypothesis that adrenergic mechanisms might play an important counterregulatory role during hypoglycaemia of greater severity, either moderate (plasma glucose nadir 2.8 ± 0.1 mM) or severe hypoglycaemia (plasma glucose nadir 2.0 ± 0.1 mM) was induced in 8 normal subjects by intravenous infusion of insulin at the rate of 25 or, on a different occasion, 50 mU/m2·min for 60 min, with and without intravenous infusion of propranolol and phentolamine, to block the effects of catecholamines mediated by alpha- and beta-adrenergic receptors. Compared to the control studies, alpha- and beta-adrenergic blockade did not deteriorate glucose recovery either during moderate, or during severe hypoglycaemia. It is concluded that in such a model of acute, insulin-induced hypoglycaemia, adrenergic mechanisms play a small, if any, couterregulatory role, regardless of the severity of the plasma glucose nadir reached during hypoglycaemia.
