A case of the association between a conspicuous moya-moya syndrome and a thalassemic syndrome is reported. The vascular disease was perfectly tolerated and discovered in relationship with a serious traumatic accident. The evaluation of the still controversial theories concerning the pathogenesis of large cerebral vessel abnormalities in sickle cell anemia could suggest the existence of this association in other hemoglobinopathies such as thalassemia.
