There is considerable confusion about the meaning of the term "bloating" and its use is frequently inappropriate. Bloating is the subjective sensation of abdominal distention, associated or not with objective distention of the abdomen. Several pathophysiological aspects are still unknown but alterations of abdominal gas production, intestinal motility and visceral sensitivity seem to have a major role. Different mechanisms may be responsible for bloating onset and treatment should be tailored on the characteristics of the individual patients: probiotics and non-absorbable antibiotics represent a promising tool for reduction of intestinal gas production, intestinal prokinetics may be useful but we are still awaiting confirmation of the clinical usefulness of new drugs for the modulation of visceral sensitivity.
