It is well known that the superior pole of osteoarthritic femoral head is histologically characterized by the presence of bone sclerosis. On the other side, the supero-medial and supero-lateral areas are characterized by bone porosis. These morphological varieties would be correlated to a different degree of bone remodeling undergone to different mechanical stress at the level of the areas above mentioned. The aim of this study was to verify and to describe the evolution of bone remodelling in weight-bearing and not weight-bearing areas of osteoartheitic femoral head using static and dynamic histomorphometry. The most interesting result we found was the enhancement of homogenous bone remodelling. It was quantitatively expressed by the increased values of static and dynamic histomorphometric parameters in the weight-bearing areas. These data seem to confirm the increasing in bone remodelling, not randomly, but strictly related to selective mechanical strength.
