Calcium antagonists and bronchial hyper-reactivity. Role of nifedipine (Adalat* * Nifedipine-Adalat®, Bayer.)in prevention of exercise-induced asthma

Abstract

The effectiveness of nifedipine in preventing exercise-induced asthma (EIA), the time course of protection and the relationship between nifedipine* plasma levels and the degree of protection were investigated. Thirty-two asthmatics (23.5 ± 7.8 years) EIA-responders, symptom free (FEV1 > 80% of predicted value) were randomly divided into two groups A and B and, using a double-blind, cross-over, within-patient design, on separate days, were given nifedipine (Adalat*) 20 mg orally or placebo. Nifedipine plasma levels were measured 60 minutes later. Two exercise challenges were then given, Group A patients one and three hours after protection, while Group B exercised at the second and fourth hours. The exercise challenge consisted of free running performed according to a standardized procedure in controlled environmental conditions. Nifedipine prevented EIA in 62.5% of Group A patients in the first test and in 56.2% in the second test (placebo: 25 and 37.5%), whereas in Group B the protection was effective in 25 and 37.5% of the patients respectively (placebo: 12.5 and 43.7%). In Group A the fully protected patients had significantly higher (P <0.025) mean nifedipine values (131.4 ± 78.8 ng/ml) than the EIA responder patients (64.9 ± 14.4 ng/ml). A significant correlation between post-exercise FEV1 values and nifedipine plasma levels was found in the nine protected patients.
