An inhibitory effect of cimetidine on the secretory activity of parathyroid cells has been observed in vitro. The effect of cimetidine administration on circulating parathyroid hormone (PTH) has been described with conflicting results, plasma PTH levels being lowered, unchanged or variable during cimetidine administration to hyperparathyroid patients. We evaluated the effect of acute administration of cimetidine on circulating PTH in six healthy volunteers. Cimetidine (400 mg) was infused over a 20 min period and blood samples for PTH and calcium assays were taken before, during and at the end of the infusion. Plasma PTH was measured with a RIA method (Cis-Sorin, Saluggia) with specificity for the carboxy-terminal region of the hormone. Plasma PTH concentration did not change significantly either during cimetidine infusion or after withdrawal of the drug. Total and ionized serum calcium were also unaffected.
