Abstract

Aim. The ideal system for classifying fractures should offer an exact description of the causal mechanism and morphology of the lesion, and characterize the degree of stability of the injured vertebral segment. The concept of stability is important in the management and prognosis of thoraco-lumbar fractures, which explains why most classifications are based on stability. Classification. The Denis classification into four groups is the most widely used (by flexion, by compression or burst, by flexion-distraction, and fracture-dislocations). Other, more detailed classifications, like that of Magerl, have not achieved the same acceptance. Imaging diagnosis. Imaging diagnosis relies on the correct use of conventional radiology, CT scan, and MRI. Initial treatment. The initial management of these patients is described in protocols for transportation, the initial stabilization, and pharmacological treatment in the first 24 hours. There is still no scientific evidence for recommending emergency decompression surgery, except in thoraco-lumbar fractures that course with incomplete neurological lesion and exhibit progressive deterioration. Monitoring. Intraoperative neurophysiological monitoring guarantees the decrease of lesions that may appear during the manipulation, reduction, and fixation of thoraco-lumbar fractures.
