Abstract

The aim of this study is to evaluate the use of a non invasive ultrasonographic technique (Echo-Doppler Duplex Scanning (EDDS)), alternative to scintigraphy and phlebography, for the early identification of subclavian vein thrombosis during total parenteral nutrition (TPN). Sixteen patients (12 M,4 F; mean age 49 ± 8 yrs) have been studied during TPN. Sixteen polyvinylchloride (PVC) catheters have been introduced through percutaneous puncture of subclavian vein. Mean indwelling time of the catheter was 20 days (10-40). 99mTc-scintigraphy and phlebography were carried out in all patients after 10-20 days from catheter insertion; EDDS exam was performed one week after catheter insertion and then every 7 days for all the study period. Nine cases of subclavian vein thrombosis (5 massive, 4 mural) were diagnosed (56%). Scintigraphy end EDDS identified all cases of thrombosis while phlebography was useful in identifying only the massive thrombosis. In conclusion EDDS has the same sensitivity as scintigraphy for the early diagnosis of subclavian vein thrombosis. Moreover EDDS is more sensitive than phlebography, it does not carry risks for the patient and it can be easily repeated.
