Investigations of the pelvic floor require a multidisciplinary cooperative approach. The aim of our study was to define a diagnostic protocol based on different neurophysiological tests that have been devised to examine the functional integrity of the afferent and the efferent nervous pathways to the pelvic floor. These techniques were performed in addition to a radiographic examination by the defecography. To this purpose we report the results obtained in three patients affected by faecal incontinence. Electrophysiological investigations included sacral reflex latency, sensory evoked potentials after anal and dorso-genital nerve stimulation, motor potentials evoked by cortical and lumbo-sacral root magnetic stimulation, electromyography of anal sphincter. The cases described are representative of neurogenic impairment at different level of the afferent and efferent pathways between brain and pelvic floor, identified by the proposed electrophysiological and radiological protocol.
