Abstract
Unstable thoracic and lumbar spinal fractures requiring surgical treatment have often a low functional outcome. Animal studies have reported an improved neurological outcome in early decompressive surgery. Clinical studies are controversial. Early fracture fixation is associated with less complications, shorter hospital and ICU staying but the neurological outcome is debated.
