N2 - The authors report a case of extramedullary plasmacytoma with a gastric localisation referred to their attention at the Division of General Surgery and Colon-Proctology of IRCCS Policlinico S. Matteo in Pavia. Having outlined the rarity of the neoplasm and its hypothetical etiopathogenesis, the authors dwell on the anatomopathological, histological and immunohistochemical characteristics, with special reference to differential diagnosis, and undertake a critical analysis of the various morphological aspects. A number of considerations are made on the symptoms and clinical conditions, highlighting the acute and massive hemorrhage that required emergency total expanded gastrectomy. The authors also underline that demolitive surgery is still the elective form of management, but its efficacy is correlated to early diagnosis and the limited extension of the neoplasm. They also point out that the rapid recurrence of disease was determined by the particularly aggressive nature and spread of this neoplasm. The surgery, which was extensively demolitive, was effective in controlling the hemorrhage in progress.
