The Authors describe a surgical case report of 'focal nodular hyperplasia of the liver' and they discuss it with reference to other studies and experiences reported in literature. Specially they point out the morbility and underline the perplexities still existing in the diagnostical and differential characterization of this pathology. The more sofisticated morphological researches like the TC, the RM and the angiography gave important data, but never the surgical exploration nor the extemporary hystological examination specified the difference respect to hepatomas. The few news about the natural history of focal nodular hyperplasia, the doubts referring to its etiopathogenetic theories and its identification are reflected on the attitude and realization of the therapy. The therapy often persist like an unsolved problem that press the search of a rationale for the opportunity, typology and cronology of treatment, in spite of the surgical resection hasn't any alternative in case of voluminous symptomatical masses in young people.
