Food-dependent exercise-induced anaphylaxis: Diagnostic value of in vivo and in vitro allergologic tests

Abstract

Eighteen patients with suspected Food-Dependent Exercise-Induced Anaphylaxis (FDEIAn) were administered prick tests with commercial food extracts (SPT) and prick by prick tests with fresh foods (P+P), and their total IgE levels and specific IgE for 31 foods were measured (CAP System- RAST) in order to identify all the food allergens that might be responsible for the attacks. The control group comprised 21 athletes with no symptoms of food allergy. On the basis of skin tests and RAST, 6 patients were positive for at most two foods and 12 for a great number of them. It is likely that 15 patients had a specific FDEIAn and the other 3 a non-specific FDEIAn. Each of the three tests used (SPT; P+P and RAST) displayed different degrees of sensibility. In some cases, the CAP System proved to be more sensitive than in vivo tests. We emphasize the importance of allergologic testing with an extensive panel of food allergens (including foods used for seasoning, such as garlic and onions) in all patients with suspected FDEIAn. We feel that the positive predictive value of our work-up is increased by using both in vivo (SPT and P+P) and in vitro (CAP System) tools.
