Small bowel tumors are very rare, representing 1-2% of gastroenteric neoplasms, although jejunum and ileum correspond to 80% of the enteric surface area. Among malignant tumors occurring in the small bowel, non epithelial types are rarely represented (as opposed to benign tumors), and those having stromal differentiation are less common still. In a short space of time we have observed two cases of GIST; the peculiarity of the diagnostic management and the histological characteristics of these cases are very interesting. The first case regards an 80-year-old male with uncommited type GIST metastatizing in a loco-regional lymphnode, treated by ileum resection. The second case concerns a 64-year-old female affected by a gastric smooth muscle type epithelioid variant GIST, treated by gastro resection sec. Roux. Both patients are enjoying good health and no signs of relapse have been observed during follow-up. The aim of this report is to describe and analyse the characteristics of these rare tumors; in classifying GIST we suggest to use the terminology introduced by Rosai in 1996; in personal opinion it is the first clear and reproducible classification scheme which resolves some of the difficulties arising from the plethora of pre-existing classification terminology, used to define the histotype of gastrointestinal stromal tumors.
