We performed an observational study to evaluate the incidence of epileptic seizures and epilepsy after stroke and to identify potential risk factors. Among 1143 patients (884 ischemic strokes, 259 cerebral haemorrhages), 80 (7%) developed acute symptomatic seizures. In the 92.5% (n = 74) epileptic seizure occurred within one week from stroke. Analysis of data revealed that a cortical site of the stroke and the severity of clinical feature were associated with post-stroke epilepsy and that the occurrence of seizures after stroke is a negative prognostic factor. Our data confirmed that metabolic changes occurring after stroke are important determinants of symptomatic epileptic seizures.
