The authors report the case of a 36 year old patient with a recurred basal-cell carcinoma of the back. Despite a wide excision in the last two recurrences, the tumor recurred again with involvement of the paravertebral muscles and of the dorsal vertebrae, thus making further radical treatment impossible. Histological examination of the neoplasms, excised between 1985 and 1990, showed that, although the edges of the excision were not involved, there was perineural infiltration of tumoral cells. The authors discuss biological and therapeutic circumstances likely to promote the aggressivity of the basal-cell carcinoma.
