The authors report a case of primary retroperitoneal subphrenic echinococcal cyst. Abdominal CTscan associated with the specific hemagglutination test were the fundamental exams for diagnosis. After several months of medical therapy with mebendazole 2 g/die, the patient underwent surgical operation with complete removal of the cyst; histologic study confirmed the diagnosis of hydatid disease. The postoperative course was uneventful. The authors, with a review of the current literature discuss the possible complications that a retroperitoneal echinococcal cyst can give.
