The exact nature of some pigmented genital macules is still unknown. Our purpose is to describe the leukomelanodermic lesions of penile lentiginosis. Methods. We studied the clinical, histological and ultrastructural features of 30 cases of penile lentiginosis. Results. Unlike previous descriptions that concentrated on dark melanotic macules, we observed that all our cases exhibited varying degrees of both hyper and hypopigmentation. Conclusions. We consider penile lentiginosis a distinct disorder mucosal melanosis, an acquired variegated variant. Because penile lentiginosis is suggestive of in situ melanoma of the genitalia, it is important to rule out the possibility of such a tumor by several biopsies and long term follow-up.
