Character and incidence of local pelvic-perineal recurrences in 203 patients treated by radical surgery for rectosigmoid cancer were investigated in order to obtain useful data for preventive purposes. A comparison between surgical intraoperative macroscopic staging (sTNM) and final histology (pTNM) showed that the first one can be considered, in the Author's experience, a reliable method to detect the amount of tissue to be removed. Casuistry analysis showed an evaluation mistake in the relapsed cases that can significantly justify the local recurrence because of a wrong intraoperative staging in default.
