Current controversies regarding distal hypospadias repairs are reviewed. The importance of a careful preoperative evaluation in conjunction with flexibility in providing different technical solutions are the keys to surgical success. Three types of repairs are discussed (advancements, extensions and tubularizations) based on the management of the native urethra and the urethral plate. Important aspects of post-operative care, including dressings and urinary diversions are also discussed.
