The methacholine challenge testing is a very informative test broadly used in respiratory medicine. In this review, starting from the recommendations from the main Guidelines, elements characterizing the test and influencing the results are considered, as: indications and contraindications, technical aspects and executive protocols. Moreover considerations about data presentation, categorization of bronchial responsiveness and interpretation of the results are reported.
