Several techniques for genitals reconstruction in 'female to male' trans-sexual patients have been described. Since 1988, 12 neophalloplastics in trans-sexual patients have been executed at the Plastic and Reconstructive Surgery of the Cancer National Research Institute of Genoa. In ten cases a tunnelized inferior pedicle rectus abdominis muscular flap has been used in perineal area. In two cases this flap has been covered with some previously expanded greater lips of pudendum skin; as to the remaining cases microsurgical technique has been used (radial flap). Neourethra reconstruction has been executed with skin graft in three cases; in 4 cases neourethra has been supplied by a rectus abdominis muscular flap, retrieved with a peritoneum flap and sutured around a catheter; in 3 cases an arm free lateral flap has been isolated. In two cases radial free flap has been used for both penis and neourethra reconstruction.
