Mid-dermal elastolysis is a rare acquired pathology of the elastic tissue which affects mostly women of medium age. It is characterized by the occurrence of skin areas of fine wrinkles which are localized to the limbs, shoulders and legs with slow tendency to progression. The clinical manifestations are expression of damage of elastic fibers of the medium dermis: papillary and reticular dermis are not affected. Other acquired elastolysis have to be differentiated. The case of a 35-year old woman who presented fine wrinkles on the lateral aspect of both arms and shoulders is reported. The histologic examination showed fragmentation and absence of elastic fibers exclusively in the medium dermis. A diagnosis of mid-dermal elastolysis, type I, according to Trueb's classification, was made. Treatment with topic retinoic acid and glycolic acid did not show significant results.
