Abstract

For some years, the endoscopic treatment of arachnoid cysts has been proposed as an alternative to conventional fenestration surgery by craniotomy or cystic decompression by shunting. A growing number of cases have been treated endoscopically in the last decade with good results so that nowadays neuro-endoscopy can be deemed the method of choice for intra and paraventricular cysts. We describe our experience in 15 cases of arachnoid cysts operated by endoscopic technique. Patients comprised ten males and five females aged between two months and 80 years with the following cyst locations: six in the Sylvian fissure, two suprasellar, five in the posterior cranial fossa, one in the frontal convexity and one interhemispheric parietal. All patients underwent endoscopy as they had symptoms of intracranial hypertension. Cystic decompression was achieved by cystocisternostomy and ventriculocystostomy using flexible and rigid endoscopes. The operation was carried out by endoscopy alone using instruments within the endoscope canals in most cases. The controlled endoscopy technique (video-endoscopy) was used in three patients with cysts in the Sylvian fissure using microsurgical instruments inserted parallel to the endoscope. Results were satisfactory with a resolution of clinical symptoms in 11 cases (73%) whereas a reduction in cyst volume was achieved in seven patients (46%). These results were maintained during long term follow-up (between one and four years). There were no cases of mortality or permanent morbidity. The advantages and limitations of endoscopy are discussed together with indications relating to cyst location and the technique is compared with traditional treatments.
