In order to evaluate the predominance and the clinical importance of the delta infection in patients affected by chronic B hepatitis, we have examined by immunofluorescence 249 liver biopsies from 101 retrospectively selected patients. The delta antigen (Agδ) was present in 87 biopsies from 35 carriers (35 per cent), with a maximum in patients affected by cyrrhosis (55 per cent) and generally in strict association with progressive liver disease (85 per cent). Although patients with intra-hepatic Agδ more severe liver impairments than Agδ-negative patients, the amount of worsening was similar in both groups during the observation period.
