Personal experience of current thinking on oncological radicality in renal carcinoma is reported. The clinical of the condition is assessed as a function of the most advanced diagnosis techniques (CT, NMR, digital angiography) with a view to achieving careful staging to be classified in the TNM system with the addition of V for venous extension. Stress is laid on the importance of the operation in the completion of the classification of individual treated cases; it is also stated that surgical operation, if it is to achieve the aim of radicality, should proceed with all due respect for basic principles such as: 1. preliminary access to the renal hilus; 2. removal of the kidney, suprarenal gland and perirenal adipose cells en bloc; 3. retroperitoneal lymphadenectomy. The series is reported and essential moments in the technique pointed out in relation to the staging of each case.
