We describe a case of a young woman, with no history of migraine, who developed, after a brain injury, migraine with aura and epilepsy. The development of migraine with aura is quite rare in chronic posttraumatic headache. In this case, it's very interesting consider the coexistence of aura, epilepsy, and SPECT features. Although, to this day, no common physiopathogenetic theory has been able to explain the correlation between cranial trauma and post-traumatic migraine and epilepsy, some authors have advanced the theory that the trauma could act as a trigger for metabolic-ionic alterations, such as the spreading depression, which could justify the cortical activation at the basis of epilectic crises and, on the other hand, the activation of the vascular-trigeminal system and hence of the pain pathways.
