A 15 years old girl underwent a conservative intervention consisting of isthmovaginal anastomosis after excision of the vaginal septum, the atretic cervix and the uterine adenyomyoma by the vaginal route. Plastic reconstruction, using an adaptation of Sturmdorff's method, restored normal menstrual cycles, without dysmenorrhea. There was a 5 years follow-up. Ultrasonography suggested the persistence of adenomyomatous tissue in the bladder detrusor, that was completely asymptomatic. Abdominal cerclage before or at the time of zygote intrafallopian transfer is advisable.
