Abstract

At a time when considerable attention is being paid to the functional recovery of patients with laryngeal tumours, sub-total laryngectomies are very much one of the available options: they are able to solve problems that cannot be handled using laser surgery, compete with organ conservation protocols and guarantee correction when these other therapies fail. The aim, as obvious as it is opportune, is to lend substance to the growing conviction that total laryngectomy must only be considered a last resort surgical option to be confined to cases of extremely late observation that are beyond other treatments. The present paper attempts to supply the state of the art, focusing attention on crico-hyoido-epiglottopexy and on crico-hyoidopexy, on present indications, possibility of integration, and the technical measures perfected in recent years in which these operations have been carried out with greater regularity, devoting particular attention to problems of a functional nature, in the light of today's knowledge on the subject and results extrapolated from personal experience and from the literature.
