Abstract

The authors report a technique of regional anesthesia of lower limb based on the block of the sciatic, femoral, lateral cutaneous of the thigh and obturator nerves. All blocks were performed with the aid of a peripheral nerve stimulator. Results concerning 28 patients who have undergone orthopaedic and trauma surgery using external fixation devices are illustrated. Optimal anesthesia was obtained using 1.5-1% mepivacaine with 1:400.000 adrenaline, such as to allow application of thigh tourniquet as well a good muscle relaxation. In no case narcosis was required. The authors observed fairly stable respiratory and cardiovascular parameters, as well as low occurrence of intra and postoperative complications. In addition, the low invasiveness of the technique, along with preservation of consciousness, sped postoperative recovery and facilitated nursing. The authors therefore recommend the use of this technique in all elderly high-risk patients referred for knee and leg surgery.
