Abstract

A diabetic 74-year-old woman came to our observation because of rapidly progressive paraparesis. Spinal MRI with gadolinium showed at D10-D11 level an extradural mass extending to vertebral foramina, with contrast enhancement as a neoplastic tissue. Neurosurgical exeresis was succesfully performed and at pathological examination the mass turned out to be histiocytic proliferating tissue, labelled as "Rosai-Dorfman" type. This rare disease is an histiocytic neoplastic disorder usually limited to lymphatic tissue, but sometimes localized only to extra-lymphatic tissue, as skin and skull bone. The literature reports extremely sporadic examples of histiocytic growth in CNS or in the spinal dura mater, like our case. Isolated serum elevation of PCR or amyloid protein may be the unique systemic sign of the neoplastic proliferation.
