Abstract

Thirty six severely symptomatic patients aged less than 5 underwent intracardiac repair for tetralogy of Fallot. Three of them, who had clinical, hemodynamic and angiographic characteristics of pulmonary atresia with ventricular septal defect, were included in this series because they had evidence of rudimentary anatomic continuity between the right ventricle and pulmonary artery. They all died. Out of the remaining 33 patients, 22 underwent primary repair, and 11 had secondary correction (1 death). Contraindications for primary repair in infancy are considered to be coronary anomalies which preclude outflow patching, and severe hypoplasia of the distal pulmonary arteries. Otherwise, primary correction at any age is the treatment of choice for every symptomatic patient with tetralogy of Fallot.
