Groin is one of the most frequent site of hernia occurrence. Aim of surgical therapy is the reduction and reconstruction of the anatomic integrity of the abdominal wall. The evolution of surgical techniques permits to improve long term follow-up and quality results. Relapsed hernia is not always a technical defect but could be referred to an aponeurosis relaxatio. In this case report the large amount of tendineal and aponeurosis loss is the main cause of relapsing. We experienced the use of new stitches and not reabsorbable mesh to obtain a definitive repair of this large hernia. Stitches have been positioned in the iliac bone to fix the mesh in a solid state, the upper part of the mesh has been anchorated in the safe and free fascia. 24 months follow-up demonstrated good positioning of the mesh in absence of hernia recurrence.
