The surgical treatment of the vulval carcinoma is frequently source of serious reconstructive problems. In the past the velvectomy was always followed by a simple closure of the surgical wound, with the poor result of a 'flat' perineous, only with the outlets of the urethral and the vaginal canals. This was the origin of big psycological problems, not only in young patients. Recently various techniques have been proposed, for vulvar reconstruction after vulvar carcinoma, among which the use of island groin flaps, reported by the Authors, in order to obtain the best morphological and functional results.
