Abstract
A patient affected by a severe COPD and hypoxemic chronic respiratory insufficiency under a telemedicine program requested a telephone contact showing signs of exacerbation with saturimetric worsening: he sent saturimetric tracings via a modem system which confirmed the relapsing situation. Seven telephone calls sheduled by a nurse tutor, 3 requests for saturimetric tracing, a change in drug therapy from a pulmonologist, a contact with the General Practitioner, the recovery from acute event with a new clinical stability is described. In conclusion, clinical and instrumental monitoring via a telematic program for patients affected by severe COPD and hypoxemic chronic respiratory insufficiency and the home care of a severe relapse is feasible if a well conducted program of telemedicine, a daily nurse tutoring, a saturimetric tracing and a pneumological second opinion are available.
