The exact recurrence rate of endometriosis after conservative surgery is not known. Rates from 2% to over 50% have been reported. The recurrence rates after conservative surgery by laparotomy and laparoscopy are quite similar, varying from 2% to 51% and from 2% to 33%, respectively. Up to now, prospective, randomized studies indicating whether laparotomy is better than laparoscopy for treating conservatively recurrent endometriosis are lacking and the choice of surgical approach depends on the operator's experience and skill. In any case, the complete removal of the disease and the restoration of pelvic anatomy are essential. According to our experience in this field, surgical options are suggested.
