Abstract
The use of free mucosa graft in urethral reconstruction was introduced for the first time by Memmelaar in 1947. However, experience of other authors was not satisfactory and this operation was disregarded for many years. Recently, progress aud refinement in surgical techniques for hypospadias repair have consistently modified results and made success more likely. Personal experience in 15 complex urethral reconstructions is reported. Every surgical step is described in detail in order to codify the technique and to obtain more reliable and standardized results. It is emphasized that indications for free mucosa grafts are confined to severe forms of hypospadias which cannot be corrected with a pedicled patch or tube of preputial skin. In fact, this technique represents a modern advancement in 'one step' hypospadias repair but, like other difficult operations, it is burdened by shortcomings and operative unpredictability and is, therefore, restricted to more experienced and skilled surgeons.
