A case of a 42-year-old female with severe reno-vascular hypertension in left solitary kidney is presented. The angiogram showed complete aortic barrage (L3 level) associated with stricture of the left renal artery and splenic artery. Right renal artery was completely occluded. A transdiaphragmatic aorto-renal by-pass with PTFE graft was performed through a thoraco-abdominal approach. The post-operative period was uneventful and patient discharged after 10 days. The renogram at 6 months from the operation shows an excellent perfusion of the kidney, the patient is in good condition and, with the same pharmacological therapy, hypertension is much more easily controlled. In highly selected patients, with severe damage of the aortic wall or splenic artery stricture, the by-pass between thoracic aorta and renal artery may be one of the best technical solutions.
