The malunited articular fractures of the wrist can be compatible with a good functionality. Surgical treatment is required only for directional defects of the distal radial epiphysis and for the alterations of the distal radio-ulnar joint. It will be carried out after considering such factors as age, job, functional deficiency, time elapsed from fracture. Younger and more active patients usually benefit from a wedge radial osteotomy, joined to bone graft. In the treatment of distal radio-ulnar lesions, the technique of Kapandji-Sauve gives better results than the resection of the ulnar head. In arthrosic painful outcomes, instead, palliative operations, such as denervation or arthrodesis, are indicated.
