Tuberous breast reconstruction using anatomical implants: A case report

Abstract

The aesthetic involvement of tuberous breast deformity has sometimes great effect on the psychological status of the patients. Different surgical procedures have been performed to face the problems related to this deformity: augmentation mastoplasty, mastopexis, combined augmentation mastoplasty and mastopexis, glandular reshaping. Notwithstanding a standard procedure to correct this deformity hasn't been established. This paper reports a 15-year-old girl affected by a bilateral third degree tuberous breast characterized by serious areola hypertrophy. The deformity was deeply impairing the relational life and the psychological growth of the patient. Our personal surgical approach consists in three accurate incisions of the mammary fascia, performed following a vertical, horizontal and oblique pattern, to expand the glandular tissue enough to fit the prostheses. Placing beneath the muscle anatomical prostheses was possible to realize a good projection and a bigger volume of the inferior portion of the breast; it enables the creation of an anatomical breast shape which represents a reconstruction following aesthetic goals. Even if postoperative areolar displacement occurred, the patient denied any secondary correction because was extremely satisfied of the result. Combining a correct surgical procedure and the use of an anatomical implant, applying a mathematical approach, enables outcomes not achievable in the past.
