Abstract

The aim of the study is to evaluate the effectiveness of a retinoid (4-HPR) in preventing recurrences and new occurrences after radical excision of basal cell carcinoma of the head and neck. During recent years, Vitamin A and/or its derivatives have been assumed to play an important role in preventing some neoplasia. The differentiating, antiproliferative and immunostimulant actions of vitamina A and its derivatives have been studied and published for many years. Since the first observation on vitamin A and its derivatives were made, showing their essential role in the growth and normal cellular differentiation, a careful study has been conducted on the more actual antiblastic effects; in fact it has been noticed that retinoids prevent or reduce some cellular responses from activating cancerogenics. The recruitment of patients will take place on the physician's and / or specialist's request: these patients must have non-treated basal cell carcinoma of the head and neck. Patients will then be examined, as 92% of basal cell tumors are situated on the face, the excision will be carried out at three millimetres from the macroscopic margin of the neoplastic lesion and only in the case of positive histologic response will the patient be include in the study provided that the is found to be eligible for inclusion. Patients will be treated with 4-HPR (200 mg / day with a three day holiday from the drug at the end of each month for one year) and will be checked after the study periods as follows: a clinical follow-up: every six months for a period of 5 years from the data of excision, blood samples and chemistries and ophthamological questionnaire every six months. The control group will undergo clinical examinations, blood and chemistries tests every six months until the end of the fifth year from the excision date.
