Primary prevention: Intensive correction of major cardiovascolar risk factors is recommended for atherothrombotic stroke in the elderly; in presence of atrial fibrillation warfarin is recomended when other risk conditions coexist; in their absence, aspirin is appropriate. When thromboembolic and bleeding risk conditions co-exist; the use of warfarin and/or aspirin should be evaluated individually. Prevention of recurrence: Antiplatelet agents prevent recurrence in elderly patients without cardiac rhythm abnormalities; when the latter are present, warfarin is the drug of choice.
