For the first time it is reported on vertebral body necrosis after phenol sympathectomy. This techniqne, established in the treatment of chronic ischemia of the limbs, in this reported case caused a collapse of the L3 body, due to necrosis, with consecutive lumbar instability and neurological deficits. The stabilization by titanium plate and autogenous bone grafting showed postoperatively a complete healing. In the author's experience the surgical treatment should be performed following the mechanical criteria which are valid in the therapy of neoplastic osteolysis.
